The owner is responsible for the decent, safe, and sanitary condition of the site, building, mechanical areas, common areas, and units.

1. Move-In Inspection
The Unit Inspection Checklist will be used to document the condition of a unit prior to or on the move-in day. The owner and tenant will sign the form and submit the document to AHFC as part of the move-in packet.
2. Annual Inspection
The Building Inspection Checklist will be used to document the condition of the property each year. The owner will keep a master file with at least three (3) years of inspections.
3. Units Not Decent, Safe, and Sanitary
During a re-inspection of previously failed items, the inspector shall use the Date Approved section of the form to indicate correction. Upon correction, each item marked fail will be initialed and dated by the inspector.
4. Forms
The following are specific instructions related to completion of the AHFC forms required for use by the owner.
4.1 Unit Inspection Checklist
This is form ADL10u.
4.1.1. Page 1, Top Section
The address for the property is listed underneath the form name.
- Inspection Type check the type of inspection
- Building Information this information does not change. If there are updates, the owner must contact AHFC to update the form.
- Unit Number enter the number or apartment number of the unit being inspected
- Inspection Date enter the date on which the inspection occurs
- Owner/Agent Name and Address this information is filled in by AHFC.
- Telephone Number enter the appropriate telephone number of the person to contact if there are questions regarding the inspection
- Inspection Results once the inspection is completed, select an option
- Circle the pass P or fail F option for the inspection for the following:
- Smoke Detector
- Carbon Monoxide
- Fire Extinguisher
- Inspector Printed Name enter the first and last name of the inspector
- Re-Inspection Pass Date if the unit initially failed, enter the date the unit passes inspection here
4.1.2. Areas to Inspect
- Select pass P, fail F, or not applicable NA for each item in these sections
- If there are comments that need to be noted, complete on each line
- Date Approved if an item fails and is later passed, enter the date the item passed here
4.1.3. Page 2, Notes
Enter any items of note or concern here if additional space is needed to explain.
4.1.4. Page 2, Signatures
- Inspector Signature and Date the inspector will always sign and date at the completion of the inspection
- Tenant Signature and Date
- Move-In Inspection - the tenant is only required to sign the move-in inspection. The owner will give a copy to the tenant for his/her records.
- Other inspection types the tenant may sign if present; it is not required. A copy will be provided by the owner to the tenant at their request.
4.2 Building Inspection Checklist
This is form ADL10b.
4.2.1. Page 1, Top Section
The address for the property is listed underneath the form name.
- Inspection Type check the type of inspection

- Inspection Date enter the date on which the inspection occurs
- Re-Inspection Pass Date if the unit initially failed, enter the date the building passes inspection here
- Owner/Agent Name and Address this information is filled in by AHFC.
